Need Help Adjusting A Singer 97-10

If the picture loads, you can see where once in a blue mon, at random intervals the bobbin thread comes all the up thru the leather??? I have the bobbin adjusted as tight as it will go, and the top backed way off on both adjustments? the one before the "wax pot" which has been removed, and the hammer type place where this is routed...

I am also having trouble with a skipped stitch on occassion! I have the foot pressure adjusted as far as it will go down but still out of the blue it skips?

I have a 24 nedle with 277 thread top and bottom!

what am I missing???

I went over some things, and found one thing that "might" be the problem! Just need input from others familiar with this machine!

Looking at the picture, where my finger is pointed, I found this nut to be loose! This is the "hammer" piece I was refering to which seems to work as a "brake" on the thread as it cycles?

HELP!!!
