The tension of the belt directly on the sheath is what keeps it in place. A friend gave me a book on traditional and old style leather projects. That style knife sheath was very popular with the Native Americans, as it allowed them to carry the knife on their sash (no belt, . . . more of a sash thingy) for use when needed, . . . and when they sat down (no chairs, on the ground) they could just tip it one way or the other, . . . out of the way so they didn't cut themselves.
